When ordering by mail, please state the following:

- Who the gift certificate is for

- Who the gift certificate is from

- Your name, address, and phone number, and where you would like the gift certificate sent

- The dollar amount or specific massage you are requesting,

- Where you would like the gift certificate sent (if different from your address)

- Date gift certificate is needed by

- A check or money order made out to Mountain Massage for the amount of the gift certifcate plus $1 for postage and handling.
